Elavil Therapy For Use in Interstitial Cystitis


Although only one drug has been FDA approved specifically for interstitial cystitis, Elmiron, treating interstitial cystitis has always been a "brand off" approach. "Brand off" is where drugs not specifically designed for the illness or disease are used because of their side effects. For example, interstitial cystitis patients are given anti-anxiety drugs, antidepressants, antispasmodics, pain medication, and even some antiseizure medication.

Elavil is the brand name for the drug amitriptyline. The other brand name for the drug is Endep . It is given to mental health patients to elevate mood by increasing the neurotransmitters in the brain. Typically, this drug is prescribed for more than one reason to interstitial cystitis sufferers. It is available in tablet form in the dosages of 10mg, 25 mg, 50mg, 75mg, 100mg and in 150mg.

Sleep Effects


Elavil tends to make one drowsy after taking, and therefore is typically prescribed to be taken at night before bed. For this reason it is given to interstitial cystitis sufferers to help them sleep through the night without having to wake up to go to the bathroom multiple times. Also, even if one doesn't typically get up at night, it will still give them a better night sleep and will help them feel better throughout the day because they were able to rest. These nerves are super sensitive and fire very easily at very low input levels; Elavil helps to elevate these levels so they don't fire so easily at low levels."

Pain Effects


Elavil will give a slight boost to treating and dealing with pain. It isn't a pain medication, but it does "take an edge off". This makes living with interstitial cystitis a bit easier. Its sedative effects help with the pain of the bladder. It raises your pain threshold so that lower level pain isn't felt as much, and you are much more able to cope.

Warnings


There are many drug interactions with Elavil. If you are taking an MAOI (monoamine oxidase inhibiting drugs) you may get a high fever, convulse, or even die while on Elavil / Endep.

If you have seizures or are at risk for having a seizure, you should not take this drug.

You should not take Epinephrine (what is known as an Epipen, given to those with severe allergic reactions) while on this drug as it raises your chance of severe high blood pressure. If you have risk factors for allergies severe enough that you carry an Epipen you need to let your doctor know prior to filling this drug prescription.

Relafen Medication - Uses and Side Effects


Relafen Generic Name is nabumetone.

Relafen is in a group of drugs called nonsteroidal anti-inflammatory drugs (NSAIDs).Relafen is used to treat pain or inflammation caused by arthritis. Relafen works by reducing hormones that cause inflammation and pain in the body.RELAFEN (nabumetone) is a naphthylalkanone designated chemically as 4-(6-methoxy-2-naphthalenyl)-2-butanone. Relafen is acidic and has an n-octanol:phosphate buffer partition coefficient of 0.5 at pH 7.4.Free fraction is dependent on total concentration of 6MNA and is proportional to dose over the range of 1,000 mg to 2,000 mg. It is 0.2% to 0.3% at concentrationsNabumetone itself is not detected in the plasma. Approximately 35% of a 1,000-mg oral dose of nabumetone is converted to 6MNA and 50% is converted into unidentified metabolites which are subsequently excreted in the urine. 6MNA is more than 99% bound to plasma proteins. 6MNA exhibits pharmacokinetic characteristics that generally follow a one-compartment model with first order input and elimination.

Relafen medication should not be used certain medical conditions. Relafen cause stomach bleeding. Curcumin, Coumadin, and Other Anticoagulants


If you see a physician for treatment of congestive heart failure and/or atrial fibrillation (A-fib), chances are you also take clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), or warfarin (Coumadin), or a similar anticoagulant. And since all anticoagulant medications work within a narrow range, you don't want either to increase or decrease your clotting factors without changing the doctor working with you to get the right dosage of your medication.

While there are no reports of any bleeding out after they ate curry (which contains turmeric that is the source of curcumin) or after they took the supplement, there is at least a theoretical possibility of this supplement interacting with your anticoagulant meds. Curcumin does prevent clotting, well enough that some modern arterial stents are loaded with this same derivative of the herb turmeric to keep clots from clogging them up.

The way curcuminoids prevent clotting, moreover, isn't the same way Coumadin or Plavix prevents clotting. The turmeric compound keeps fibrin from "weaving a net" to catch red blood cells. Coumadin prevents the liver from making the fibrin "fibers" in the first place. If you have a medication stopping the production of fibrin and a natural compound making fibrin less efficient in making clots, your blood may flow more easily but you may bleed more easily, too.

If you are taking an anticoagulant, have a discussion with your doctor before you take curcumin. This way you can work together proactively to make sure you get the maximum of both your prescription medications and any supplements you take.

Rimonabant And The USA


The day of reckoning for 2007 has now come and gone for the FDA to review the medication rimonabant. Rimonabant is also known as Acomplia in Europe and other countries and Zimulti was then name it was to have in the United States. June 13th, the FDA voted against recommending this medication to hit the USA pharmacies. What does this mean for Sanofi-Aventis and what does the future of this medication look like for the United States?

Rimonabant has been past by many European countries for the treatment of obesity. It works differently than most other diet medications as well as has some possible benefits. Basically put, Rimonabant works by blocking receptors in the brain that are responsible for food cravings. When you’re eating less, it means generally you are consuming fewer calories and thus you lose weight. Some of the other potential benefits of this drug are that it has been shown to increase the “good” cholesterol while reducing “bad” cholesterol and has shown promise for helping smokers kick the habit.

While we don’t know all that went on in the FDA meeting, we do know that some of the issues that where brought up about this medication. One of the big obstacles they had issues with was its possible psychiatric side effects including suicidal influences. The FDA panel was not satisfied with the amount of feedback they had regarding this medication. And most likely they will keep an eye on it including watching the feedback coming in from other doctors around the world. It looks likely that a 50 month trail will then determine if this medication will be accepted into the USA market, a trail that would not be complete until 2010. The FDA had been scrutinized in the past for allowing drugs to hit the USA pharmacies in what some considered a too short of a study period. I suppose with such a possible popular medication such as Acomplia, that the FDA did not want to overlook all aspects of this medication before ever accepting it in the USA.

Eosinophilic Esophagitis: Diagnosis and Treatment of Allergic or ...


How is eosinophilic or allergic esophagitis diagnosed?

It is first suspected by the classic history and the appearance of the esophagus on endoscopy of multiple rings or constrictions that result in the esophagus looking like that of a cat’s esophagus. This is called felinization of the esophagus or ringed esophagus. Other visual signs may be whitish spots, long furrows or a lining that looks like crepe paper and is very easily torn.

The definitive diagnosis is made by the finding of an abnormal number of eosinophils in esophageal biopsy tissue. The number of eosinophils considered abnormal is debated between >15-25 eosinophils per high power field (400x) with most authorities agreeing >20 as being diagnostic. The esophagus normally contains no eosinophils. It has been long accepted that chronic acid reflux commonly can be associated with 5-10 eosinophils per high power field in the lower esophagus but this finding higher in the esophagus should raise likelihood that eosinophilic esophagitis is present.

Why is it important to diagnose eosinophilic esophagitis and distinguish it from acid reflux esophagitis?

Acid reflux esophagitis usually responds to acid blocker medications like histamine 2 blockers ranitidine (Zantac), cimetidine (Tagamet), famotidine (Pepcid) and proton pump inhibitors like omeprazole (Prilosec) or esomeprazole (Nexium) whereas eosinophilic esophagitis often does not get better with these medications. Acid reflux injury to the esophagus can result in narrowing or constriction of the lower esophagus causing a food sticking sensation. This can be treated by a stretching of the constriction known as esophageal dilation that is usually safe and highly effective. Eosinophilic esophagitis also may result in constrictions of the esophagus but the stricture or rings are usually multiple, located higher in the esophagus and carry a high risk of tearing or puncturing the esophagus if dilation is attempted before treatment with steroids. There does not seem to be an association of cancer of the esophagus with eosinophilic esophagitis like there is with acid reflux. However, eosinophilic esophagitis can be chronic and difficult to treat.

How is eosinophilic esophagitis treated?

Identification and elimination of problem allergy foods is the mainstay of treatment. Foods that show positive allergy testing are eliminated. In some people a strict elimination diet is recommended. Rarely, a diet of only basic amino acid proteins in a liquid (elemental diet) is required. Temporary relief can be achieved with steroids. Systemic (oral prednisone) works but has the potential side effects of steroids on the rest of the body. Topical steroids applied directly to the surface that have little or no absorption into the blood stream are preferable. The nasal steroid, fluticasone propionate, has been used successfully. It is sprayed in the mouth and swallowed twice daily. The mouth should be rinsed out followed by spitting out rather swallowing the water. No eating or drinking for 30 minutes is recommended. All You Need To Know About Antidepressants


Many people often have misunderstandings about what antidepressants are capable of.

First of all, they are not pick me up pills; they don't artificially bring on a feeling of happiness, euphoria, or unrealistic well-being. Nor do antidepressants insulate you from life, make you not care about vital things, or make you oblivious to sorrow or loss.

What antidepressants do is avert depressed persons from sliding into the blackest depths of depression when something awful happens. They can still feel wounded, pain, and apprehension, but they feel these the way people normally do when they don't have depression. They also can help depressives sleep soundly, increase their energy, and improve their ability to concentrate.

The way antidepressants work is interesting. There are two chemicals, serotonin and norepinephrine, that have to do with the transmission of impulses between nerve cells in the brain and seem to be allied with depression. It would appear as if depressed people use up these chemicals at an accelerated rate than other people. Antidepressants help to retain these chemicals, apparently leading to feelings of reduced anxiety, more security, increased self-worth, assertiveness, and resilience.

There are a mixture of types of antidepressants, but they fall into a number of straightfoward categories. These are tricyclics, MAOIs, and lithium, and the newer medications: hetereocyclics and Prozac and Prozac-related drugs.

Until rather recently, tricyclics were the normal treatment for depression. These medications include imipramine (Tofranil), amitriptiline (Elacil), Vivactil, Norpramin, Pamelor, and Sinequan. Still in conventional use today, 40 to 70 percent of depressed patients improve substantially with tricyclics.

Although they are quite efficient medications, there are some negatives to their use. They generally take several weeks of constant administration to be successful, which is difficult to handle when people are sincerely distressed. Also, it is relatively easy to take a disastrous overdose. In general, tricyclics should only be used on a short-term basis. They are not addictive, but they must be used with care, especially with people who have cardiovascular disease.

Monoamine oxidase inhibitors (ie. MAOIs) include Marplan, Parnate, and Nardil. They are a different class of drugs and cause a different reaction in the brain. These drugs are helpful from some people who do not respond to tricyclics. These drugs can have disagreeable side effects, but the main disadvantage of MAOIs is that they can also cause a stroke if certain foods containing the compound tyramine (cheese, red wine, pickles) are consumed while they are being used.

Lithium is generally the treatment of choice for bipolar disorder (the cycle of manic highs with depressed lows). In the correct dose, lithium reduces by about 50 percent the chances of another manic episode within a year. Mood swings become fewer, shorter, and less harsh. The success rate for lithium treatment is 70 percent, and 20 percent of people become symptom-free. It is generally seen as a maintenance drug. Once the patient is on Lithium, they are on it for life.

One of the most well-known drugs on the market today is Prozac. It has been followed into the marketplace by many other new antidepressant medications like Zoloft and Paxil, two near cousins, Effexor and Serzone, and some more distant cousins, notably Wellbutrin, Desyrel, and BuSpar.

Unlike tricyclics, which affect the levels of both serotonin and norepinephrine in the brain, Prozac, Zoloft, and Paxil affect only serotonin. Hence they are known as selective serotonin reuptake inhibitors, or SSRIs, meaning that they prevent or slow down the reabsorption of serotonin. Effexor and Serzone affect both serotonin and norepinephrine, and the others have more complex effects. All, however, have been shown to be effective in the treatment of depression. The choice of which of these medications to use for a particular person has to do with their dosage and side-effect profile. Paxil, for example, seems to have a soothing effect on anxiety that Prozac lacks. Effexor has the reputation of being more energizing than Prozac.

Compared with tricyclics, the side effects of Prozac and its cousins are usually small. Tricyclics can give you dry mouth, make you constipated, and actually slow you down, whereas Prozac has none of these problems and gives you a little more energy. However, there are some side effects with the newer antidepressants which should be mentioned. Most notable among these is a reduction of interest in sex and complications maintaining an erection. Although the male performance problems usually go away after a few weeks, many people on SSRIs report a continued diminished interest in sex, which can certainly add to marital problems.
